Injury To Achilles Tendon

Some times some people do not avail a chance and let that go. A folk tale in Orissa,India describes how people ignore the opportunities.Once “Lord Shiva and His wife Goddess Parbati“were in the sky to observe the people of earth.They saw a poor man in skeletons begging for food but was not getting anything.Parbati became felt pity for him and asked Shiva to see this and also asked why people created by them should suffer like this.God replied to it with a smile that some do not like a grab an opportunity and are lazy ,consequently suffer.And He wanted to show that to Goddess Parbati then and there.The god put a pot full of gold coins on the road of the beggar .But the beggar at that moment wanted to mimick a blind man and walked closing his eyes.He gave a kick to the pot and walked past to open his eyes later .Consequently he missed the gold filled pot.Then the god asked his wife to interpret it and they went on the voyage.

Something of the sort happened to-day in our hospital.A person injuring his Achilles tendon came to me.Since ours is a private hospital only for port trust employees ,other people have to pay for the service with a token price only.I inspected the injury and asked him to deposit the required fees to get the tendon repaired .But the person expressed his inability to pay.I suggested him if he does not have the money he can pay it later also.He was not willing for that also.But on humanitarian ground I offered him to repaired it free of cost using material we get as physician’s sample but he has to get admitted to the hospital for some time.And to my utter surprise he declined for that and went away. Instantly my staffs reminded me of the story I just narrated.
The thick tendon through which the calf muscles get anchored to the heel bone is called Achilles tendon named after the Greek mythological hero Achilles.This is a tough tendon in the body.This tedon lifts the body weight off the ground while walking.

This tendon gets severed mainly by accidents,assault or in collagen diseases.When partially torn the person can walk limping and disability is more severe if completely torn.The person can not stand on toes of the affected foot.And if completely severed a distinct gap can be felt over an intact skin or the end can be seen if skin is cut.The upper end of severed tendon usually is dragged by the calf muscles to an upper level and may not be seen.Tendon repair is a special procedure and done under aseptic conditions .It can be performed under local.regional or general anaesthesia.The repair is done using a non-absorbable suture material.After repair a plaster of paris cast is given to immobilise the foot for 6 weeks.After that period a gentle physiotherapy makes the patient fit for duty.The results are good.

THIS DAY(SUNDAY) A HOLIDAY/WHY I LEFT ORISSA GOVT.SERVICE.

The climate GOD, at last become happy to make the mankind in this part of India feel the cool of Rainy season.It is pleasant now.To-day is a holiday but emergency section in hospital is on as usual. Now it is in news that Govt. want to double the salary of doctors willing to work in remote localities. I do not know whether this will yield any result.Basic question is not only salary.It all about infrastructure and other facilities.In remote areas there is no good residence facility,if one is there it may not be electrified and if that is there no provision of water supply.Something will be in shortfall.If all these are there no good facility (school) for study of their children.If all of the above are there there will be lack of proper equipments for smooth  professional work and growth.  After completion of study one doctor always wants to utilize his knowledge in treating patients and in want of the facility he draws  flak. Also there is no full proof transfer policy,Policy for career progression, policy for promotion and above all low payment in comparison to other less stressful jobs.
  It is not that the so called think thaks do not know it ,it is their mental set up not to give better facility to doctors . Govt. should short out these problems before relying too much  only on hiking the pay to draw doctors to remote areas.
All these are causes for why I left Orissa Govt. service.

HUMANIZED GOD

The lord of Lords Shree Jagannath alongwith his elder brother Lord Balabhadra and sister Subhrada reside in Puri,Orissa.They are more loved than worshiped and everybody in this part of world feel the lords as one among themselves. And that is the beauty of the Lords.The rituals performed are more like a mortal human being than God. To be more clear the God follow the life style of human i.e. They take birth, eat,sleep,travel,dress up neatly for various acccasions fall sick and misunderstood by” Maa Lakshmi” ( Wife of lord Jagannath) and also leave this mortal world after a certain time and also buried.Again New bodies for the Lords are prepared of sacred wood and rituals performed as if they take birth called “Nabakalebar” that means new body or birth.
So the Lords had gone to their aunt’s house some 9 days back and will return to their sanctum-santorium to- day. But have to pacify Maa Lakshmi as they have not taken her along with them. This is in very very short about to-day’s ritual called the return journey “Bahuda Jatra”.
They are mingled in the blood and flesh of each Oriya for their humanised way of living and are irreseparable.Now the world also recognoses it and people from all over the world will be seen here.
  Why I write this is for a cause.Because according to definition of health “health is a state of physical,mental and social well being and not merely the absence of disease or infirmity” and later on to it spiritual well being could be added.

THE HOLY CAR FESTIVAL IN PURI.

Lord Jagannath, Orissa

To-day is the auspicious occasion the Car Festival of the Living Gods, the adored deity of the Oriyas and the people world wide, The Jagganath ,Balabhrada and Sister Subhadra.This will be celebrated in Puri,Orissa.They are the God of mass though worshiped as Hindu God.They will travel to their aunt’s house and en- route bless the mass by their presence. There is no restriction to-day for offerings .All are busy to make the mega event run smoothly in the presences of millions of devotees.The largest kitchen of the world of the Gods’ here will be busy in serving the hungry.The mega event will be broadcasted worldwide with a lot of affection and love.It is said that whoever will see the deities on the “Rath”,the car ,will not have re-birth.
I shall be watching the event in my TV,to-day being a holiday for us.Will you like to…!!

HOT AND HUMID CLIMATE/HEAT STROKE

It is very much hot and humid here in Paradip,Orissa,India.The monsoon is scheduled to arrive.During my walking sweat comes out from scalp as rain drops.But I completed it.I have scheduled some endoscopies to-day at 8 AM.I hope the day will not be better without rain.This is the difficulty in coastal areas.
  People are prone to heat stroke during this time, a fatal outcome in these areas .T hope this is not seen in cold countries.I lot of fluid and avoiding exposer to sun are the mainstray of prevention.Govt of Orissa has closed the schools ,prevented labourers from working in the mid-day and installed road side water supply to the comuters.

  Let us stay safe.

CUTTING NAIL AND BRUSHING TEETH

I do not know who named the hurricane”Aila” but in oriya it maens “came” and that came and went away leaving behind it’s mark in form of devastations in Orissa and West Bengal in india and Bangledesh.That also weakened the the advancing south west monsoon to delay it’s arrival in advance to this part of world.And it is extremely hot and humid here.I cut short my walking time by five minutes due to unbearable heat and sweating.My fingers are sweating to make the keys wet.
So now if I ask you how do you cut your nail and brush your teeth, it will sound silly.But believe me there is 90% chance that you may end up in giving a wrong answer.Many come to me with ulcers on their nail bed to be diagnosed as in-growing nails may it be finger or toe.That is due to faulty trimming of nail.Long nails look nice in the fingers especially in females.
Unfortunately the nail ends are cut in a semi-circular manner in many giving rise to corners going deep in to the nail bed .That causes an in-growing nail and consequently an ulcer.So it should cut in a straight manner well out side the pulp or nail bed so that the corners are well out side the nail bed and can not bury into it.That has a good look also.

My second question must get answered .Teeth should be brushed  starting  from the gingival side to the tip of teeth and repeated  all the times so that the dirt comes out.If done in a reverse manner all the dirt go into the gingival fold and raises the fold from the teeth .That may result in tarter or calculus deposit around the teeth.Brushing should be at least twice a day after meals and a soft brush be used and changed if damaged .There is no need to emphasize that a good quality tooth paste is also required.
